Medical Transportation
Chatham Transit Network is offering a medical transportation program, Health Rides, for county residents age 60 and older and adults age 18 and older with a disability or health condition that prevents them from driving. This includes all physical, mental and emotional disabilities, including MH, DD, SA and any physical disability that prohibits someone from providing their own transportation. The program will help them get to medical and dental appointments and pharmacies.
It is offered as a voucher program, which means that an individual or agency, such as Freedom House, Therapeutic Alternatives or El Futuro, must provide a request to Chatham Council on Aging on behalf of the Chatham resident needing the service. The eligibility form is a 1-page, simple form requesting the transportation and providing details such as date, time, address, destination and a contact number.
The request form also will validate that the person in need of transportation is a disabled adult (temporary or permanent, any disability that places an individual in need of transportation) and so should be signed by an MD or a licensed clinician. The requesting agency is responsible for maintaining a record of all requests that Chatham Council on Aging may access in the event of an audit.
